Job summary

AccentCare in Reading, PA is seeking a Physical Therapist, Home Health to evaluate and treat patients as part of an interdisciplinary team, with thorough documentation in line with policies and standards. You will manage a caseload, participate in conferences and in-services, and contribute to patient education and outcomes.

The role covers Wyomissing, PA with a 40-hour workweek, salary range $98,280.00–$109,200.00, and benefits including medical/dental/vision, PTO, and 401(k).

Qualifications

• Master's degree in physical therapy approved by CAPTE accredited physical therapist education program.
• Minimum nine months experience as a physical therapist.
• Preferred community/home health experience.
• Currently licensed to practice as a physical therapist within the state of agency operation.
• Must possess and maintain valid CPR certification while employed in a clinical role.
• Must be a licensed driver who can travel to all business locations.

Responsibilities

• Provides patient care and documents treatment according to physician orders and patient needs.
• Performs patient evaluations in accordance with agency policy and procedure.
• Informs the physician of assessment results and recommended interventions.
• Develops and documents treatment plans and establishes priorities.
• Provides education and counseling to patients and families.
• Presents relevant patient information to physicians and team members.
