You will assess, plan, and deliver skilled OT services following state and federal guidelines and payer requirements. You will collaborate with clients, families, and the care team to promote independence, safety, and quality of life, document progress, and adjust plans of care as needed.

Qualifikationen

• Graduate of an Occupational Therapy program accredited by ACOTE.
• Must be licensed and in good standing in Colorado.
• Strong communication and documentation skills.

• Provides skilled OT services following standards and regulations.
• Maintains patient safety and HIPAA compliance in home health.
• Develops and updates Plan of Care with MD orders as needed.
• Collaborates with clients, families, and the care team to support functional goals.
• Documents outcomes and progress in a timely, accurate manner.
