Job summary

Optimal Home Care is seeking an Occupational Therapist (OT) to deliver high-quality OT services to patients in Colorado. The OT will follow AOTA standards, ensure safety and privacy, and collaborate with physicians to develop and implement individualized plans of care.

The role emphasizes family-centered care, professional ethics, and adherence to regulatory requirements while coordinating with Nursing, PT, SLP and other team members to optimize patient outcomes.

Qualifications

• Graduate of an OT program ACOTE-accredited.
• Licensed to practice OT in Colorado.
• Strong communication and interpersonal skills.

Responsibilities

• Provides occupational therapy services per AOTA standards and payer requirements.
• Maintains patient safety and confidentiality per HIPAA and regulations.
• Develops and implements Plan of Care with patient goals and MD collaboration.
• Documents interventions, outcomes, and progress within required timeframes.
• Coordinates care with other agency disciplines to support patient needs.
• Ensures documentation and approvals are completed promptly.
