Job summary

Optimal Home Care is seeking an Occupational Therapist (OT) to provide high-quality OT services to patients in Colorado. The OT will follow AOTA standards, ensure patient safety, HIPAA compliance, and collaborate with the client and MD to develop and implement a Plan of Care.

The role requires current Colorado OT licensure, strong communication and interpersonal skills, and the ability to document outcomes and coordinates with other agency disciplines to optimize patient independence and quality

Qualifications

• Graduate of an Occupational Therapy program accredited by ACOTE or its predecessors.
• Must be currently licensed and in good standing in the state of Colorado.
• Must be able to communicate in the language of the client and document client related activity as appropriate.
• Possesses adequate and effective written and verbal communication skills.
• Possess adequate and effective interpersonal skills.

Responsibilities

• Provide occupational therapy services in compliance with AOTA standards and payer requirements.
• Maintain patient safety and HIPAA compliance in practice.
• Develop, document, and implement Plan of Care in collaboration with the client and MD.
• Collaborate with clients and interdisciplinary team to address daily living activities and goals.
• Comply with home health regulations and agency policies and procedures.
