Job summary

OneWell Health Care is seeking a Flexible Schedule Licensed Practical Nurse (LPN) to provide skilled nursing services in home and community settings across Pennsylvania. The LPN will work under an RN Case Manager, implementing care plans, administering medications, and supporting individuals and families in health, safety, and quality of life.

Responsibilities include wound, catheter, feeding tube care, vital signs, and documentation in compliance with regulatory standards.

Qualifications

• Current and active Licensed Practical Nurse (LPN) license in Pennsylvania.
• CPR and First Aid certification (required).
• Completion of an accredited Practical Nursing program approved by the Pennsylvania State Board of Nursing.
• Minimum of one (1) year nursing experience required.
• Experience supporting individuals with IDD, home health, or long-term care preferred.

Responsibilities

• Implement individualized care plans and administer prescribed medications and treatments.
• Perform clinical procedures including wound care, catheter care, feeding tube management, and vital sign monitoring.
• Educate and train families, caregivers, and new nursing staff; participate in interdisciplinary meetings.
• Maintain infection control and safety practices in care environments and ensure timely documentation.
