Job summary

Lifeline of Jackson Purchase Home Health, part of the Optum family, is seeking a Licensed Practical Nurse to provide high-quality in-home patient care according to the plan of care. You will document visits in the EMR and coordinate with the health care team to ensure safe, effective care.

Requirements include an active LPN license in the state, CPR certification, a valid driver's license or dependable transportation, and the ability to work flexible hours to meet client needs.

Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Current CPR certification.
• Current driver's license and vehicle insurance, access to dependable vehicle, or public transportation.
• Ability to work flexible hours as required to meet identified client needs.
• Ability to function in diverse home settings and populations.

Responsibilities

• Provides high quality clinical services within the scope of practice as defined by state laws and the plan of care, coordinating with the health care team.
• Documents patient visits thoroughly in the electronic medical record per regulatory and payer requirements.
• Participates in care coordination, reporting changes in condition and communicating with patient, family, physician, and care team.
