Optum's CHRISTUS HomeCare - Tyler unit is seeking a Licensed Practical Nurse to deliver quality in-home care under the plan of care and collaborate with the health care team.

Requires current LPN licensure in the state, a valid driver’s license, and access to reliable transportation; CPR certification preferred and home health experience is a plus.

Responsibilities

• Coordinate care with patient, family, physician and care team; adapt to patient needs.
