Job summary

Safe Life Home Health is seeking a dedicated Home Health Aide / CNA to join our Chicago-area team. You will assist patients with daily activities, monitor vitals, manage medications reminders, and support overall well-being in a home health setting.

The role requires an active CNA certificate, CPR certification, a valid driver’s license, and comfort with EMR systems. Excellent communication and genuine compassion are essential for providing high-quality care.

Qualifications

Active CNA certificate required.
CPR certification recommended.
Valid driver’s license and car insurance required.
Prior home health experience is a plus.
Familiarity with EMR Kinnser by WellSky advantageous.

Responsibilities

Assist with Activities of Daily Living (ADLs) such as bathing, dressing, grooming and toileting.
Provide mobility support and assist with transfers and walking.
Take and record vital signs (blood pressure, pulse, temperature).
Give medication reminders and ensure adherence to schedules.
Assist with meals and monitor dietary restrictions.
Maintain patient hygiene and comfort; change bed linens and oral care.
Observe and report changes to the nursing team.
Document care and vitals in the EMR (Kinnser by WellSky).
Communicate clearly with patient, family and healthcare team.
Follow infection control and safety protocols to ensure patient safety.
Provide transportation to appointments as needed.
Offer emotional support and companionship.
Collaborate with nurses and therapists for comprehensive care.
Educate patients and families on self-care and safety.
