Job summary

Safe Life Home Health Care seeks dedicated CNAs in Chicago, IL to join our team and provide compassionate home health support. You will assist patients with daily activities, monitor vital signs, and help with mobility and medication reminders.

The role emphasizes patient comfort, safety, and collaboration with nurses and therapists to deliver comprehensive care in patients' homes. A CNA certificate and CPR are required, with drivers license preferred.

Qualifications

Active CNA certificate or license.
CPR certification required.
Excellent communication skills and compassionate demeanor.
Valid driver’s license and car insurance.
Familiarity with EMR Kinnser by WellSky is advantageous.

Responsibilities

Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and toileting.
Support mobility including walking, transferring, and use of mobility aids.
Take and record vital signs and monitor health status.
Provide medication reminders and ensure adherence to schedules.
Prepare meals and assist with feeding according to dietary restrictions.
Maintain hygiene and comfort; change linens and provide oral care.
Document care provided and changes in patient condition in EMR.
Communicate clearly with patients, families, and healthcare team.
Follow infection control and safety protocols; transport to appointments as needed.
Offer emotional support and companionship to patients.
