Job summary

OneWell Health Care is seeking a dedicated Personal Attendant to provide hands‑on care in home and community settings in La Verne, CA. You will assist with activities of daily living, mobility, medication support, and safety, following individualized care plans and regulatory requirements.

Eligible candidates have a high school diploma, CPR/First Aid certification, and previous DSP or caregiver experience. A valid driver’s license and reliable transportation are required for participant

Qualifications

• High school diploma or equivalent preferred.
• CPR and First Aid certification required or must be obtained within designated timeframe.
• Experience working with IDD and Autism is a plus.
• Ability to use electronic documentation systems.

Responsibilities

• Assist with activities of daily living, personal care, and mobility.
• Provide medication support as authorized and promote independence and safety.
• Observe health changes, document accurately, and report to supervisors.
• Transport participants when authorized and maintain required documentation.
