Job summary

A Place at Home is seeking a dedicated Home Health Aide to join our at‑home care team. Under the supervision of registered nurses, you will support clients with daily living activities and help maintain comfort and safety in their homes.

You will perform ADLs, assist with meals and medications, provide light housekeeping, mobility support, and compassionate companionship. A state‑approved HHA certification and a clean background check are required to deliver high‑quality care.

Qualifications

• Completion of state-approved Home Health Aide certification course.
• Must pass a background check and drug test.
• Must have valid Home Health Aide certification.

Responsibilities

• Assist clients with ADLs (bathing, dressing, grooming, toileting).
• Plan and prepare meals, observe dietary restrictions.
• Provide medication reminders per care plans.
• Perform light housekeeping (dusting, vacuuming, laundry).
• Assist with mobility and transfers, ensure safety.
• Provide companionship and nurturing relationship.
• Document and report changes to supervising nurse.
• Follow HIPAA and protect client PHI.
• Participate in ongoing training and professional development.
