Job summary

A Place at Home is seeking a Home Health Aide to support clients in their own homes under registered nurse supervision. You will assist with ADLs, mobility, and daily tasks to maintain comfort and safety, while fostering trust and independence.

Ideal candidates hold a state-approved HHA certification, can pass a background check and drug test, and have a high school diploma or equivalent. Reliable transportation and a caring attitude are essential for success in this role.

Qualifications

• Completion of state-approved Home Health Aide certification course.
• Must pass background check and drug test.
• High school diploma or equivalent required.

Responsibilities

• Providing assistance with ADLs, such as bathing, dressing, grooming, and toileting.
• Assisting with meal planning and preparation, observing dietary restrictions when necessary.
• Assisting with medication reminders in compliance with established care plans.
• Performing light housekeeping tasks such as dusting, vacuuming, and laundry to maintain a clean and comfortable living environment for our clients.
• Assisting with mobility and transfers, ensuring client safety at all times.
• Providing companionship and fostering a nurturing relationship with our clients.
• Documenting and reporting changes in client's physical condition, behavior, or living conditions to the supervising nurse.
• Following HIPAA guidelines and maintaining the privacy of all client PHI (Personal Health Information).
• Participating in ongoing training and professional development opportunities.
