Job summary

SM Home Health Care seeks a dedicated Home Health Aide to provide personal care and support under the direction of a Registered Nurse or licensed therapist. The aide will be assigned to specific clients and perform tasks to maintain client comfort and safety in the home setting.

Responsibilities include personal care, ROM exercises, assistance with ambulation, meal preparation, medication assistance under RN supervision, and clear documentation of care provided.

Qualifications

• Successful completion of a formal certification training program/written skills test and competency evaluation.
• Be at least eighteen (18) years of age.
• Minimum of six (6) months work experience in a supervised setting, preferably a health care facility.
• Demonstrated ability to read, write and follow a written Plan of Care.

Responsibilities

• Performs personal care activities, including bathing, shampooing, oral hygiene, shaving, and dressing.
• Assists in ambulation or exercises and range of motion activities under supervision.
• Performs household services essential to health care at home, including meal preparation and feeding.
• Assists in administration of medications ordinarily self-administered under RN direction.
• Reports changes in client condition to the Registered Nurse and documents care provided.
• Maintains infection control, safety, and confidentiality; participates in required in-service programs.
