Job summary
Holy Name Medical Center in Hackensack, NJ, is seeking a dedicated Home Health Aide to assist patients with daily living activities and rehabilitation goals. You will work under the guidance of registered nurses and physical therapists to provide compassionate, skilled care in patients' homes or hospital settings.
Responsibilities include personal care, transfers, rehab support, incontinence care, and accurate documentation.
Qualifications
- High School Diploma or GED required.
- Certified Home Health Aide certification preferred or required where applicable.
- NJ Driver's license valid upon hire.
Responsibilities
- Assist patients with personal care including bathing, grooming, and hygiene.
- Safely transfers patients to bed, chair, or commode and maintains comfort.
- Support rehabilitation exercises as directed by Nurse or Therapist.
- Provide bowel/bladder care and assistance with toileting as needed.
- Record intake and output accurately and document weekly assignments.
- Operate a Hoyer lift safely under supervision.
- Maintain patient confidentiality and comply with care plans.
- Communicate patient status changes to the Case Manager promptly.
