Job summary

Holy Name Medical Center in Hackensack, NJ seeks an experienced Home Health Nurse case manager to coordinate and oversee Home Care/Hospice services.

You will perform physical assessments, develop care plans with goals and timelines, document diligently, and work with patients, families, and referral sources to ensure continuity of care. A nursing license in NJ and 1–3 years of hospital or home care experience are preferred.

Qualifications

• Diploma in Nursing required or higher.
• RN license in New Jersey required.
• 1–3 years hospital or home care experience preferred.
• Good physical assessment and documentation skills.

Responsibilities

• Coordinate and case manage Home Care/Hospice patients.
• Perform initial and ongoing nursing assessments.
