Job summary

Hackensack Meridian Health in New Jersey is seeking an in-home Homemaker/Home Health Aide to support patients with personal care and daily living activities. The role emphasizes compassionate service and adherence to care plans under RN direction.

Responsibilities

• Provide in-home personal care under the RN direction.
• Notify RN of changes in patient condition and document promptly.
• Record and document all required patient information per protocol.
• Assist patient with transfers and ambulation; perform ROM exercises.
• Perform light housekeeping and change bed linens; assist family with light chores.
• Assist with self-administration of oral medications and remind dosing.
• Prepare simple meals per dietary instructions and assist with eating.
• Assist with ostomy care and catheter hygiene; change ostomy bags; perform peri care.
• Perform other duties as assigned; remain flexible across tasks.
• Adhere to organizational competencies and standards of behavior.
• Lifts/pushes/pulls up to 50 lbs; stands up to 8 hours daily.
