Job summary

Bridge Home Health is seeking a dedicated Hospice Aide for the Central Coast (Salinas, CA) to join our home health team. Under RN or therapist supervision, you will deliver personal care, observe and report patient conditions, and help ensure safe, compassionate care in the patient's home.

This full-time role requires valid California Driver’s License and CPR, with responsibilities including assisting with meals, toileting, mobility, documentation, and adherence to care plans.

Qualifications

• Empathy for the needs of the ill, injured, frail and impaired.
• Ability to read and follow written instructions and document care given.
• Self-directing with the ability to work with little direct supervision.
• Demonstrates tact, patience, and good personal hygiene.
• Must possess a valid California Driver’s License with an automobile that is insured and in good working order.

Responsibilities

• Assisting the patient with toileting activities.
• Planning and preparing nutritious meals.
• Assisting in feeding the patient, if necessary.
• Taking and recording temperatures, pulse, respiration, and blood pressure.
• Assisting in ambulation and exercise according to the plan of care.
• Reporting on the patient's condition and significant changes to the assigned nurse.
• Adhering to care procedures and standards of personal conduct.
