Job summary

Compassionate Care, serving multiple California counties, is seeking a Hospice Home Health Aide based in Merced County. The role requires one year of hospice experience, a CA Home Health Aide certificate, and a valid CA driver’s license.

You will follow care plans, assist with daily tasks, and support patients in a compassionate team environment. Compensation ranges from $21 to $23 per hour, negotiable based on experience, with full-time benefits and a focus on patient-centered care across home

Qualifications

• Medical Terminology is required for this role.
• A keen understanding of normal aging and health care for older adults is expected.
• Excellent communication and listening skills, particularly via the phone.
• Strong attention to detail and ability to manage processes.

Responsibilities

• Follows a plan of care established by a primary nurse.
• Reports changes in patient’s condition to the Case Manager.
• Maintains safe conditions in the Residence and reports unsafe conditions.
• Bathes patient and provides personal hygiene care.
• Makes bed and maintains a clean environment for patients.
• Performs light housekeeping tasks essential for a safe environment.
• Attends required in-service programs and team planning meetings.
• Prepares meals and assists with feeding when necessary.
• Assists patients with prescribed exercises.
