Job summary

K-day PACE in Los Angeles, CA is seeking a CHHA/CNA to provide direct personal care for participants in homes, the PACE center, and community settings under RN supervision.

You will assist with ADLs/IADLs, observe changes, and support participants in staying safely in the community. This position requires CA CNA and CHHA certificates, BLS/CPR, a valid CA driver’s license, and immunizations.

Qualifications

• High school diploma or equivalent is required.
• Current, active California CNA and CHHA certificates issued by CDPH.
• Current BLS/CPR certification required.
• Valid California driver’s license and proof of auto insurance.
• Minimum one year of experience with frail or elderly populations.

Responsibilities

• Provide direct personal care and ADL/IADL assistance as outlined in the Plan of Care.
• Safely transfer and ambulate using DME with proper body mechanics.
• Support restorative care and home programs under RN, PT, or OT guidance.
• Monitor skin integrity and report changes promptly.
• Document care and observations in the electronic health record.
