Job summary

Team Select Home Care is seeking a Home Health Aide (HHA) to provide skilled personal care and ADLs in patients' homes under RN supervision. You report to the Registered Nurse and work within established care plans and agency policies.

Responsibilities include assisting with daily living activities, documenting timely visit records, and maintaining infection control. Must have a HS diploma/GED and CPR certification; state licensure may be required.

Qualifications

• Must have High School Diploma or GED.
• Must be 18+ years old or per state regulation.
• Maintain CPR certification per state requirements.
• Maintain licensure or certification per state regulation.
• Strong communication and compassionate care required.

Responsibilities

• Provide personal care and ADLs as directed by RN.
• Assist with medications ordinarily self-administered per state regulation.
• Perform household chores, meals, and other delegated tasks.
• Complete visit records in a timely manner per agency policy.
• Report changes in patient condition to the RN.
• Maintain infection control practices and safety protocols.
• Attend staff meetings and care conferences.
• Follow agency policies and maintain confidentiality.
