Job summary

Team Select Home Care is seeking an in-home caregiver/ aide to provide medically supervised personal care and related tasks in accordance with the plan of care. This role supports patients in their homes, performing delegated duties and maintaining strict infection control practices in collaboration with RNs and other team members.

Candidates should have a High School Diploma or GED, CPR certification, and the ability to read, write, and follow directions.

Qualifications

• High School Diploma or GED completed.
• 18 years of age or per state regulations.
• Maintains CPR and health certifications per state requirements.

Responsibilities

• Provides services as ordered in the plan of care/aide care plan and that the aide is permitted to perform under State law.
• Performs/assists with direct patient personal care, meals, and household tasks.
• Performs simple procedures as an extension of nursing services as assigned.
• Assists in ambulation or exercises as assigned.
• Assists with medications that are ordinarily self-administered as assigned per state regulation.
• Maintains infection control practices, especially handwashing and precautions as directed by the RN.
• Completes appropriate visit records in a timely manner.
• Reports changes in patient condition to the RN promptly.
