Responsibilities

• Provides personal care/hygiene to the home health patient.
• Assists with transfers, exercise, and ambulation.
• Turns and positions bed-bound patients.
• Assists with ROM as directed by RN or Therapist.
• Assists with meals and nutritional support.
• Manages simple non-sterile dressings per regulations.
• Maintains patient environment and linens.
• Reports changes to the case manager.
• Participates in IDT meetings and staff training.
• Follows patient plan of care and agency policies.
