Job summary

Providence at Home with Compassus is seeking a Home Health Aide to provide personal care and basic nursing support to patients in their homes under an RN clinical supervisor. You will follow a written plan of care, document patient status, and collaborate with therapists and clinicians to ensure quality home-based healthcare.

Requires CNA certification and strong communication skills; preferred candidates have 1 year of HHA experience and are comfortable working in patients' homes with attention

Qualifications

• Education to meet CNA certification/licensure requirements.
• Preferred: 1 year experience as a Home Health Aide.
• Skills: ability to read, analyze, and interpret care plans and to communicate effectively.

Responsibilities

• Provides competent personal care as outlined on the Home Health Aide Plan of Care established by the patient, family caregiver and case manager.
• Records vital signs and assists with basic nursing tasks under supervision.
• Performs light housekeeping to support patient independence and recovery.
• Communicates with the supervising nurse and care team to update the plan of care.
• Maintains patient dignity, privacy, and safety during care.
