Well Care Health seeks a dedicated Home Health Aide to provide in-home nursing care, including vitals, wound care, and patient education. You will support patients across all life stages under RN or therapist supervision and participate in creating personalized care plans while maintaining patient comfort and safety.

The role requires NC Nurse Aide certification, registration, and at least two years of relevant experience. A flexible schedule and a chance to grow with a caring team are offered.

Qualifications

• High school diploma or equivalent required.
• Completion of Nurse Assistant Certification course required.
• Current listing on NC Division of Facility Services Nurse Aide Registry required.

Responsabilités

• Provide home-based nursing care including vital signs and ADL assistance.
• Document care given in patient record and maintain accurate notes.
• Report changes to the care team and help develop care plans.
