VitalCaring is seeking a caring Home Health Aide to provide hands-on, in-home support and dignity to patients in their homes. You will work closely with nurses, therapists, and care teams to deliver personalized care and improve quality of life.

This role emphasizes compassionate patient interactions, independence, and strong time management as you follow care plans and document services in the EMR system. CPR certified and a valid license are required.

Qualifikationen

• Meets state and federal requirements for Home Health Aide certification or training.
• At least 18 years of age.
• Ability to read, follow care plans, and document services.
• Current CPR certification.
• Valid driver’s license, reliable transportation.
• Ability to work independently with strong time management.
• Demonstrates empathy, patience, and professionalism.

Aufgaben

• Provide compassionate, hands‑on care for patient comfort, safety, and independence.
• Assist with bathing, dressing, grooming, and toileting.
• Support mobility and transfers per care plan.
• Maintain a clean, safe home environment.
• Prepare meals and assist with feeding.
• Take and record vital signs as trained.
• Assist with medication self‑administration as directed.
• Document care provided accurately in the EMR.
• Participate in team meetings and care coordination.
• Provide compassionate support to patients and families during visits.
