Job summary

Team Select Home Care is seeking a dedicated Home Health Aide to provide skilled personal care and ADLs in patients' homes under RN supervision. You will report to the Registered Nurse and follow the care plan while maintaining infection control and timely documentation.

Qualifications

• High School Diploma or GED.
• Must be 18 years of age or older per state regulation.
• Maintain CPR certification and health certifications per state requirements.
• Maintain licensure or certification per state regulation.

Responsibilities

• Provides personal care, ADLs and household tasks per care plan and state law.
• Assists with simple procedures as an extension of nursing/therapy services.
• Aids in ambulation or exercises as assigned.
• Assists with medications ordinarily self-administered as allowed by state rules.
• Maintains infection control practices; follows RN guidance and agency policy.
• Completes visit records timely and reports changes toRN promptly.
• Attends staff meetings and care conferences as scheduled.
