Job summary

The Lakes Home Care, Inc. is seeking a compassionate Home Health Aide to provide daily living and personal care services in patients' homes according to a care plan.

Responsibilities include bathing, hygiene, dressing, transfers, ROM exercises, vital signs as ordered, and light housekeeping. You will report changes to the supervising nurse and maintain clear communication with the family and office.

Qualifications

• Meets all state and federal requirements for home health aides.
• Able to read, follow written instructions and document care given.
• CPR certification current and valid.

Responsibilities

• Assist patients with activities of daily living per plan of care: baths, hygiene, dressing and undressing, toileting, feeding.
• Take and record vital signs as ordered (temperature, pulse, respiration, BP).
• Assist with safe transfers and ambulation; perform ROM and simple procedures as ordered.
• Help with patient laundry and maintain safety using equipment properly.
• Observe and document patient condition and report changes to the nurse supervisor.
• Communicate clearly with client, family/caregiver and office.
