Job summary

The Lakes Home Care, Inc. is seeking a compassionate Home Health Aide to provide daily living and personal care services in patients' homes in accordance with an established care plan.

Responsibilities include assisting with baths, grooming, mobility, meal support, and monitoring vital signs as ordered, while following the plan of care and communicating with the supervising nurse and family. Requirements include CPR certification, reading and following instructions, and completing Maxim

Qualifications

• Meets training requirements per state and federal laws when applicable.
• CPR certification maintained per state regulations.
• Completes Maxim competency program as required.

Responsibilities

• Assist patients with baths, grooming, dressing, toileting, feeding and mobility.
• Take and record vital signs as ordered and document findings.
• Follow the plan of care and communicate with nurse supervisor and family.
• Promote home safety and proper use of equipment.
