Job summary

The Lakes Home Care, Inc. seeks a compassionate Home Health Aide to assist patients in their homes per the established care plan. You will perform personal care tasks and daily living activities, ensuring safety and comfort during visits.

Responsibilities include assisting with baths, dressing, feeding, vital signs, and documentation, while maintaining clear communication with clients, families, and the supervising nurse. Flexible hours and part-time opportunities are available.

Qualifications

• Must have CPR certification and be able to read/write care plans.
• Ability to follow written instructions and document care given.
• Effective communication with clients and family/caregiver.

Responsibilities

• Assist patients with daily living activities per plan of care (baths, hygiene, dressing, toileting, feeding).
• Take and record temperatures, pulse, respiration, and blood pressure as ordered.
• Assist with safe transfers, ambulation, ROM, and prescribed exercises.
• Perform patient laundry and help meet safety needs using equipment safely.
• Observe and report changes to the assigned nurse supervisor.
