Job summary

The Lakes Home Care is seeking a compassionate Home Health Aide to provide daily living and personal care to patients in their homes per an established care plan. Responsibilities include bathing, oral hygiene, dressing, toileting, feeding, transfers, ROM exercises, safety monitoring, documentation, and communicating clearly with clients, families, and the supervising nurse.

Qualifications

• Ability to read, follow written instructions and document care given.
• Maintain CPR certification in accordance with state regulation.
• Meets all training requirements in accordance with federal and state laws.
• PPD or chest x-ray if applicable.
• Successful completion of Maxim screening examinations.
• Participation and completion of Maxim’s competency program.

Responsibilities

• Assist patients with daily living and personal care per plan of care.
• Baths, oral hygiene, dressing, toileting, feeding, etc.
• Take and record vital signs as ordered and report changes to nurse.
• Safe transfer, ambulation, and ROM as ordered.
• Observe safety practices and use equipment properly.
• Communicate clearly with client, family/caregiver, and office.
• Document care and report to supervising nurse.
