Job summary

Symbii Afton Home Health & Hospice is seeking a compassionate home health aide to support patients in their homes across Wyoming. You will provide personal care, meal assistance, and help with daily activities under the supervision of a registered nurse.

The role emphasizes safety, documentation, and adherence to state regulations. A valid CPR certificate, reliable transportation, and the ability to work independently are required; compassion for the ill and elderly is essential.

Qualifications

• Meets the training requirements in accordance with State and Federal laws.
• At least 18 years of age.
• Ability to read and follow written instructions and document care given.
• Self-directing with the ability to work with little direct supervision.
• Empathy for the needs of the ill, injured, frail and the impaired.
• Possess and maintains current CPR certification.
• Licensed driver with automobile that is insured in accordance with Organization requirements and is in good working order.

Responsibilities

• Providing personal care including baths, back rubs, oral hygiene, shampoos, changing bed linen, dressing and undressing, skin care, toileting assistance, and keeping living areas clean.
• Planning and preparing nutritious meals and assisting in feeding, if necessary.
• Taking and recording temperatures, pulse, respiration and blood pressure when ordered.
• Assisting in ambulation and exercise according to the plan of care.
• Performing range of motion and other simple procedures as ordered.
• Assisting patient in self-administration of medication.
• Doing patient’s laundry, as appropriate.
• Meeting safety needs and using equipment properly.
• Reporting on patient’s condition and significant changes to the assigned nurse.
• Adhering to documentation and care procedures and standards of conduct.
