Job summary

Herewith in Thousand Oaks, CA is seeking an In-Home Dialysis Care Partner to support patients in their homes. You will ensure safe environments, prep treatments, and monitor wellbeing, working with a clinical team.

Requires at least 1 year of dialysis experience. Compensated at $25 per hour with consistent weekly hours and scheduling aligned to each care plan. Training and placement assistance are provided; you will work independently yet be supported by clinicians.

Qualifications

• 1+ year of dialysis patient support experience.
• Familiarity with home dialysis machines preferred.
• Reliable, empathetic and detail-oriented.
• Strong interpersonal communication.
• Must pass background check and vetting via Herewith platform.

Responsibilities

• Set up dialysis treatments for patients (NxStage, Tablo or equivalent).
• Provide companionship, emotional reassurance, and ensure schedule adherence.
• Monitor safety and assist with light ADLs.
• Maintain communication with clinic nurses and care coordinators.
