Herewith is seeking a compassionate Dialysis Care Partner to support a 69-year-old patient needing daily in-home dialysis supervision. Training will be provided as you assist with home dialysis sessions, monitor equipment, and ensure safety throughout treatments.

Responsibilities include setup, environment upkeep, companionship, and reporting to the nurse, with 4 days per week scheduling and 2–3 hour sessions. This role emphasizes patient dignity and steady, hands-on support at home.

Qualifikationen

• 1+ year of dialysis experience (home or clinic).
• Familiarity with home dialysis systems preferred (Tableu or NxStage is a plus).
• Open to in-home dialysis support and willing to complete the provided training.
• Steady, supportive, and patient—especially during an adjustment period.
• Strong communicator who works well with a clinical team.
• Must pass a Herewith background check.

Aufgaben

• Assist with dialysis treatment setup (Tableu or NxStage dialysis machine).
• Keep the treatment environment clean, organized, and safe.
• Offer companionship and emotional reassurance.
• Help the patient remain on schedule with treatment times.
• Provide light ADL support as needed.
• Monitor safety and report concerns to the coordinating nurse.
• Maintain simple documentation and shift notes.
