Job summary

PACE Southeast Michigan Direct Home Care Worker position provides compassionate in-home support to participants in our catchment area, assisting with daily activities, transportation, and appointments.

You will follow each participant's care plan, document visits, and report progress to the RNCM. The role requires strong interpersonal skills, reliability, and the ability to work with diverse populations in home and community settings.

Qualifications

• Minimum High School Diploma or GED required.
• Must have minimum of 2 years of relevant work experience.
• Must have completed formal training program (Direct Care, Nurse Assistant, Medical Assistant, etc.) outside of PACE SEMI in lieu of minimum years of experience.
• Participates in annual in-service training including infection control, TB testing, safety training, and BLS.

Responsibilities

• Personal care assistance with activities.
• Assistance with meal preparation / feeding.
• Assistance with toileting / managing incontinence.
• Simple treatments.
• Shopping.
• Medication reminders.
• Laundry services.
• Extended hours care.
• Homemaking services / chores.
• Transportation and off site appointment support.
• Accurate timely documentation of visits and progress.
• Report customer service complaints and deviations to supervisor.
• Respond to emergencies per policies and procedures.
• Work within established scope of practice.
