Job summary

PACE Southeast Michigan is seeking a Direct Home Care Worker to provide in-home and community-based support for frail elders, including care planning adherence, transport, and day center activities.

Responsibilities include assisting with daily living, meals, medications reminders, and housekeeping while documenting care and maintaining strong caregiver–family relationships. Must have solo and team collaboration skills and comply with safety and infection-control protocols.

Qualifications

• Minimum High School Diploma or GED.
• At least 2 years of relevant care experience.
• Completed formal training program (Direct Care, Nurse Assistant, Medical Assistant, etc.).
• Annual in-service training and screenings including infection control, TB testing, safety training, and BLS.
• Must be medically cleared for communicable diseases and up-to-date immunizations.
• Ability to relate to individuals with dementia and diverse ethnic/cultural backgrounds.

Responsibilities

• Assist with personal care and activities of daily living.
• Assist with meal preparation and feeding.
• Assist with toileting and managing incontinence.
• Provide medication reminders.
• Perform homemaking tasks and laundry.
• Provide transportation and support during appointments/outings.
• Accurate and timely documentation of care and services provided.
• Report deviations from schedule or appointments to supervisor.
• Respond to emergencies per established policies and procedures.
• Work within the established scope of practice.
