Job summary

Qualifications

• Minimum High School Diploma or GED is required.
• Must have a minimum of 2 years of relevant work experience.
• Completed formal training program (Direct Care, Nurse Assistant, Medical Assistant, etc.) outside of PACE SEMI is acceptable in lieu of minimum experience.

Responsibilities

• Personal care and activities assistance.
• Meal preparation and feeding assistance.
• Toileting and incontinence management.
• Shopping and errands as needed.
• Medication reminders and coordination with care team.
• Laundry, housekeeping, and homemaking support.
• Transportation and appointment support.
• Accurate timely documentation of care provided on every contact.
