Eminence Home Care in Bala Cynwyd, PA, is seeking a compassionate Home Care Intake Coordinator to be the first point of contact for clients and families, guiding them through the intake process and coordinating care services. In this role you will collect essential information, collaborate with clinical staff, ensure timely documentation, and maintain clear communication while upholding regulations and company policies.

Hours are Monday through Friday, 9:00 a.m.

Qualifications

• Healthcare or home care intake experience preferred.
• Excellent communication, interpersonal and organizational skills.
• Ability to handle sensitive information with confidentiality.
• Familiarity with electronic medical records and healthcare software.

Responsibilities

• Conduct thorough initial assessments to gather necessary information from clients and families.
• Facilitate the intake process, ensuring all documentation is completed accurately and timely.
• Collaborate with clinical staff to assess needs and determine appropriate service levels for clients.
• Maintain clear and ongoing communication with clients, families, and the home care team.
• Ensure compliance with regulations and company policies throughout the intake process.
• Provide clients with information about services available and support them through onboarding.
