Job summary

Action Health Staffing is seeking a Certified Nursing Assistant for home care, supporting patients in their residences. The role involves direct patient care, personal hygiene assistance, and adherence to a Plan of Care with EVV documentation.

Responsibilities include transfers, basic nursing tasks, meal preparation, companionship, light housekeeping, and accurate recordkeeping. Travel within the service area is required; medications are not administered.

Qualifications

• Maintain certification with DFS.
• High school diploma or GED preferred.
• Possess effective communication skills to read, write and follow directions.
• Ability to perform physical tasks necessary to treat clients who may include lifting, bending and standing.
• Must have sympathetic attitude toward the care of the sick.

Responsibilities

• Provide direct patient care as assigned.
• Provide personal hygiene care (bath, shave, hair care, mouth care).
• Assist with transfers, repositioning, and ambulation.
• Provide basic nursing care and prescribed exercises as instructed by supervising staff.
• Prepare meals considering client preferences and special diets.
• Provide companionship and activities.
• Light housekeeping duties.
• Maintain records required by agency.
• Record and report changes in client's condition.
• Complete EVV tasks as required.
• Demonstrate dependability by punctual reporting.
• Adapt to client and family value systems.
• Assist in promoting independence within client abilities.
• Follow Plan of Care; never administer medications.
