Job summary

Action Health Staffing is seeking a Certified Nursing Assistant to provide in-home care to patients in their residence. Responsibilities include direct patient care, personal hygiene, transfers, and basic nursing tasks under supervision.

The role requires reading, writing, effective communication, and empathy, with DFS certification maintained. Depending on assignment, expect travel within a defined area and adherence to the Plan of Care.

Qualifications

• Maintain DFS certification as required.
• High school diploma or GED preferred.
• Effective communication, reading, writing, and following directions.

Responsibilities

• Provide direct patient care as assigned.
• Provide personal hygiene care including bath, shave, hair care, mouth care.
• Assist with transfers, repositioning, and ambulation.
• Provide basic nursing care and prescribed exercises as instructed by a supervising clinician.
• Prepare meals considering client preferences and diets.
• Provide companionship and activities tailored to client needs.
• Light housekeeping duties as assigned.
• Maintain complete and accurate records per agency policy.
• Record and report changes in client condition.
• Complete EVV tasks and timesheets as required.
• Demonstrate dependability and punctuality.
• Adapt to client and family value systems.
• Support client independence within abilities.
• Follow care plan; do not administer medications.
