Job summary

Action Health Staffing is seeking a Certified Nursing Assistant to provide in-home care to patients in their residence. The role includes direct patient care, personal hygiene support, meal preparation, and light housekeeping, with a focus on dignity and comfort.

Responsibilities include monitoring client condition, documenting changes, and following the Plan of Care. The position often requires travel within a specified area and adherence to agency policies.

Qualifications

• Must maintain DFS certification.
• High school diploma or GED preferred.
• Ability to read, write and follow directions.
• Physical tasks including lifting, bending and standing.
• Sympathetic attitude toward the care of the sick.

Responsibilities

• Provide direct patient care as assigned.
• Assist with transfers, repositioning, and ambulation.
• Provide personal hygiene care.
• Prepare meals according to dietary needs.
• Provide companionship and activities.
• Light housekeeping duties.
• Maintain accurate client records.
• Record changes in client's condition.
• Follow Plan of Care; never administer medications.
