Job summary

Action Health Staffing is seeking a Certified Nursing Assistant to provide in-home care services. You will assist with daily living activities, personal care, transfers, and meal preparation at the client’s residence.

Travel within a defined area is required, and adherence to the Plan of Care is essential. Ideal candidates have a high school diploma or GED, strong communication skills, and a compassionate approach to patient care.

Qualifications

• Maintain certification with licensing/DFS as required.
• High school diploma or GED preferred.
• Good reading, writing and comprehension abilities.
• Physical capability for lifting, bending and standing as needed.
• Empathetic, patient-centered attitude toward care recipients.

Responsibilities

• Provide direct patient care as assigned in home care settings.
• Assist with transfers, repositioning and ambulation.
• Support personal hygiene and basic nursing tasks under supervision.
• Prepare meals per client preferences and dietary needs.
• Provide companionship and engage in activities per client needs.
• Record and report changes in client condition as required.
• Adhere to Plan of Care and EVV documentation as directed.
