Job summary

Action Health Staffing Home Care Services is seeking a Certified Nursing Assistant to provide services in patients' residences. You will perform direct patient care, personal hygiene, basic nursing tasks, and documentation via EVV while following care plans.

The role requires the ability to lift up to 50 lbs, reliable transportation for area travel, and strong communication to coordinate with families and supervisors. Medication administration is not included.

Qualifications

• Maintain DFS certification.
• High school diploma or GED preferred.
• Ability to perform physical tasks and lift up to 50 lbs.

Responsibilities

• Provide direct patient care as assigned.
• Assist with personal hygiene including bathing, shaving, hair and mouth care.
• Assist with transfers, repositioning, and ambulation.
• Provide basic nursing care and prescribed exercises as directed by supervisors.
• Prepare meals according to client preferences and dietary needs.
• Provide companionship and engage in activities as appropriate.
• Light housekeeping as needed and required by plan of care.
• Maintain accurate records per agency policy.
• Document changes in client condition and activities in EVV.
