Job summary

Avenues Home Care of Tennessee is seeking a compassionate Patient Care Assistant (PCA) to support clients with ADLs, medication assistance, and daily living tasks in their homes.

You will report to a supervisor, assist with lifting up to 50 pounds, and follow care plans and HIPAA guidelines to deliver safe, respectful care. We offer flexible shifts and benefits as part of a supportive team.

Qualifications

• Good communication skills and ability to follow verbal and written instructions.
• Proficiency with Homecare Company Management System—ClearCare.
• Valid driver’s license and reliable transportation.

Responsibilities

• Assist clients with ADLs as per care plan (bathing, dressing, grooming, continence care).
• Provide medication assistance, meal prep, housekeeping, laundry, shopping and transportation.
• Offer companionship and report changes to supervisor using agency system and logs.
• Adhere to all policies, safety rules and HIPAA requirements.
