Job summary

Avenues Home Care of Tennessee is hiring Patient Care Assistants to help elderly and disabled clients with daily living activities, medication reminders, and companionship in the Northeast Tennessee area. The role includes flexible shifts and on-site care at client homes.

You will document daily activities, follow care plans, and uphold safety and HIPAA standards while staying professional and punctual for assigned shifts.

Qualifications

• Work requires good communication skills.
• Ability to follow verbal and written assignments.
• Demonstrate proficiency with ClearCare Homecare Management System.

Responsibilities

• Assist clients with activities of daily living as specified in the plan of care.
• Provide medication assistance, housekeeping, laundry, meal prep, and companionship.
• Report daily activity to supervisor through the agency system and logs.
• Attend to other duties as assigned and communicate as needed.
