Job summary

Northern Arizona Hospice is hiring a Home and Community Based Services Aide to help clients remain safely at home. You will provide personal care, assist with daily living tasks, monitor health, and document changes while maintaining confidentiality and using EVV.

Responsibilities include supporting household tasks, meal prep, and accompanying clients to appointments. A valid driver’s license and reliable transportation are required; CPR and 6+ months in home care preferred.

Qualifications

• Current driver's license, vehicle insurance, and reliable transportation.
• Current CPR certification or ability to complete within 90 days of hire.
• 6+ months of home care experience.
• Ability to work flexible hours and independently.

Responsibilities

• Provide personal care and assist with daily living activities such as bathing, grooming, dressing, ambulation, and medication reminders.
• Support household tasks, meal preparation, and accompany clients to appointments or errands as needed.
• Monitor and document client condition, vital signs, and incidents; maintain confidentiality and use EVV system.
• Ensure a safe environment, operate medical equipment properly, and respond promptly to client needs.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.
