Job summary

Northern Arizona Hospice, part of the Optum family, is seeking a Home and Community Based Services Aide to support clients in Sedona and surrounding areas. You will assist with personal hygiene and daily living activities to help individuals remain in their homes.

Responsibilities include bathing, grooming, meal prep, light housekeeping, and documenting care using EVV. CPR certification preferred; 6+ months home care experience preferred; must have valid driver's license and reliable

Qualifications

• Current driver's license, vehicle insurance, and reliable transportation
• CPR certification preferred or ability to obtain within 90 days
• 6+ months of home care experience preferred
• Ability to work flexible hours and independently

Responsibilities

• Provide personal care and assist with daily living activities such as bathing, grooming, dressing, ambulation, and medication reminders
• Support household tasks, meal preparation, and accompany clients to appointments or errands as needed
• Monitor and document client condition, vital signs, and incidents; maintain confidentiality and use EVV system
• Ensure a safe environment, operate medical equipment properly, and respond promptly to client needs
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client
