Job summary

Providence at Home with Compassus seeks a Speech-Language Pathologist to plan and provide treatment for patients with communication disorders in a home health setting.

You will assess needs after referrals, document within 24 hours of visits, develop a written care plan, and coordinate care with families, physicians, and insurers.

California licensure within 30 days, delivery of SLP interventions, and adherence to Medicare/Medicaid requirements are required.

Qualifications

• Master’s degree in Speech-Language Pathology and licensure required.
• Active California SLP license within 30 days of hire.
• Preferred 1 year clinical experience as an SLP.
• Home health experience preferred.

Responsibilities

• Assess SLP needs after referrals with history and exam.
• Document assessment data within 24 hours of home visit.
• Develop and revise a written plan of care every 60–90 days.
• Coordinate care with families, physicians, and insurers.
• Deliver SLP interventions per care plan and goals.
• Ensure compliance with Medicare/Medicaid requirements.
