Gentiva Hospice is seeking an experienced RN Case Manager to lead in-home hospice care, delivering skilled nursing and pursuing individualized care plans. You will assess patients, coordinate with the IDG, and educate families to support comfort and dignity at end of life.

Role requires Florida RN license, CPR, and 1+ year experience in hospice/home health; background screening is required and benefits include comprehensive coverage and professional growth opportunities.

Qualificações

• RN with Florida license and CPR certification.
• Minimum 1 year RN experience in hospice/home health or related fields.
• Hospice or palliative care experience preferred.

Responsabilidades

• Perform comprehensive patient assessments and reassessments, with patient and caregiver education.
• Deliver nursing interventions and symptom management per care plans.
• Coordinate care across IDG and document timely progress notes and care plans.
• Educate caregivers and communicate condition changes to the care team.
