Job summary

Catholic Charities Disabilities Services in the Capital District invites applicants to join as a Supportive Health Assistant. You will assist with healthcare needs in the Residential Program, support nurses and clinicians, and coordinate with staff and families to ensure safe, compassionate care.

Qualifications include a NYS Driver’s license, a high school diploma, and at least two years as a Direct Support Professional in the I/DD field.

Qualifications

• Valid NYS Driver’s License.
• High School Diploma or GED.
• At least 2 years of Direct Support Professional experience in I/DD.

Responsibilities

• Assisting the Nurse Manager, RN’s and LPN’s with managing the health care needs of individuals in the Residential Program.
• Maintaining communication with program staff, management, providers, nurses, doctors, ER staff, case management, pharmacies, parents/guardians, advocates, school staff and agency leadership.
• Providing direct support to individuals served in the agency as needed.
• Possessing knowledge of basic medical terms, proper medication administration, and related clinical disciplines.
