Job summary

InnovAge Driver – Participant Care provides transportation for frail and elderly participants from their homes to Day Centers and appointments, and on field trips as part of the activities program. They also deliver in-home meals and handle delivery and pickup of supplies and medications for participants.

Physical attendance at a facility or participant home is essential. The role follows a Monday–Friday schedule with rotating Saturdays/holidays and requires safe driving and participant

Qualifications

• DOT and Defensive Driving experience.
• One year of experience with the frail or elderly.
• Ability to lift, push, pull and carry up to 75 pounds.

Responsibilities

• Drives InnovAge vehicles safely and follows routes.
• Loads and unloads participants and assists into/out of homes.
• Transports participants to pre-arranged appointments.
• Delivers in-home meals, supplies and medications to participants.
• Assists participants with outerwear and moving obstacles as needed.
• Keeps Dispatch updated on route events.
• Maintains vehicle safety and coordinates maintenance with the Manager of Transportation.
• Assists other drivers with loading/unloading at centers.
• Participates in Plans of Care for InnovAge PACE participants.
• Other duties as assigned.
